Terms & Conditions of Use & Session Purchase By using the Massaging Water Table and/or purchasing a single session, 5-session package, or 10-session package, you acknowledge and agree to the following terms and conditions: 1. Purpose & Nature of Service The Massaging Water Table is a wellness and relaxation service intended to support comfort, muscle relaxation, and general well-being. It is not a medical treatment and does not replace skilled therapy, medical care, or clinical services. 2. Eligibility & Age Requirement Use of the Massaging Water Table is limited to adults age 18 and older. Children and minors are not permitted to use the Massaging Water Table. Use is subject to staff approval. 3. Health Considerations & Safety Users should notify staff prior to use if they are pregnant, have implanted medical devices (such as pacemakers), active injuries, recent surgeries, circulatory disorders, or any medical condition that may be affected by vibration, heat, or massage. Circle Creek Therapy reserves the right to deny or discontinue use if the service is deemed inappropriate or unsafe for the individual. 4. Assumption of Risk Use of the Massaging Water Table is voluntary. By participating, you acknowledge and accept any inherent risks associated with its use and agree to follow all posted instructions and staff guidance. 5. Session & Package Purchase Terms Options include single sessions, five (5)-session packages, and ten (10)-session packages. Packages may be shared among immediate family members living in the same household, provided all users meet the adult eligibility requirement. All sessions and packages are non-refundable and may not be sold or transferred outside the household. Sessions have no cash value and may not be exchanged for other services. Packages must be used within the following timeframes from the purchase date: 5-Session Package: six (6) months 10-Session Package: twelve (12) months Unused sessions after the expiration period are forfeited. 6. Scheduling & Cancellations Sessions are subject to availability. Missed appointments or late cancellations may result in forfeiture of the session. 7. Payment Massaging Water Table sessions are a self-pay wellness service and are not billable to insurance. Full payment is due at the time of purchase. 8. Conduct & Use Expectations Users must remain clothed and remove shoes prior to using the Massaging Water Table. The table must be used only as instructed by staff.
